Observation

During onsite monitoring visit, licensor observed loose cable wires capable of forming a loop attached to the side of the house, computer and printer loose cords and wires capable of forming a loop, and loose hanging TV cords in the licensed play area capable of forming a loop accessible to children. (2)(c) During onsite monitoring visit, licensor did not observe placement stickers to prevent children from walking through glass, on the sliding glass door leading to the outdoor play area. (2)(f) During onsite monitoring visit, licensor observed protruding rusty nails exposed on playground barrier, several boards of the barriers with dry rot and pointed jagged edges, and pointed jagged edge of broken fence board in the outdoor play area posing a risk of cuts, abrasions or punctures accessible to children. (3)(a) During onsite monitoring visit, licensor observed a broken child play stroller in the outdoor play area accessible to children. (3)(g) During onsite monitoring visit, licensor observed outlets accessible to children in the hallway leading to bathroom, and outlets in the licensed indoor play space that do not meet the blank plated or tamper resistant requirement. (5)(a) During onsite monitoring visit, licensor observed an outlet next to the sink in the licensed bathroom not meeting the GFCI outlet type and accessible to children. (5)(b) During onsite monitoring visit, licensor observed power strips/surge protectors used to plug in computer and printer, accessible to children in the licensed space. (5)(e)
